Your Guide to Timed Intercourse
One of the biggest misconceptions about trying to conceive is that intercourse should occur when ovulation happens. In reality, conception is most likely when sperm are already present in the reproductive tract before the egg is released.
After ovulation, the egg remains viable for only 12–24 hours. Healthy sperm, however, can survive in fertile cervical mucus for up to five days, allowing them time to travel through the cervix, uterus, and into the fallopian tubes where fertilization occurs.
This is why having intercourse in the days leading up to ovulation is much more effective than waiting until ovulation has already occurred.
A landmark prospective study by Wilcox and colleagues, published in the New England Journal of Medicine, examined thousands of menstrual cycles to determine the probability of conception based on the timing of intercourse relative to ovulation.
*The findings showed:
2 days before ovulation: 25–30% (highest chance)
1 day before ovulation: 20–25%
Day of ovulation: 10–15%
1 day after ovulation: Close to 0%!

The takeaway is that the best time to have intercourse is before ovulation- not after. In fact, the single highest chance of conception occurs approximately two days before ovulation, followed closely by the day before ovulation.
This is why I encourage patients not to wait for the "perfect" moment. Instead, begin timed intercourse as soon as your body starts showing signs that ovulation is approaching.
Tracking Cervical Mucus
One of the earliest signs that ovulation is approaching is a change in cervical mucus. Estrogen causes cervical secretions to become increasingly watery and slippery as you approach ovulation, creating an ideal environment for sperm survival.
That small gush of fluid that happens mid-cycle? That can be a sign of peak fertility!
Throughout your cycle, cervical mucus typically follows this pattern:
After your period: little or no discharge
Early cycle: sticky or creamy discharge
Approaching ovulation: watery discharge
Peak fertility: clear, slippery, stretchy mucus that resembles raw egg whites
After ovulation: thicker, tackier mucus or dryness
Research has shown that cervical mucus observations correlate well with the fertile window and can help identify your most fertile days before ovulation occurs.
My recommendation: Don't wait until your mucus looks exactly like raw egg whites. As soon as you notice your discharge becoming wetter, clearer, or more slippery than usual, begin timed intercourse.
Using LH (Ovulation Predictor) Test Strips
Ovulation predictor kits (OPKs) detect the luteinizing hormone (LH) surge, which typically occurs 24-36 hours before ovulation.
Many people mistakenly wait until they have their darkest test line before having intercourse. However, by the time your test is fully positive, your fertile window has already begun (and is actually close to ending).
Here's how to interpret your LH strips:
Faint line: Normal. Small amounts of LH are always present.
Darkening line: LH is beginning to rise and ovulation is approaching.
Positive test: The test line is as dark as or darker than the control line, indicating the LH surge.
Darkest line: Often represents your LH peak, but ovulation usually follows soon afterward.
My recommendation:
Rather than waiting for your darkest test:
Start intercourse when you notice the LH line becoming progressively darker than previous days, even if it isn't yet positive.
Continue intercourse every 1-2 days through your positive LH test and for one additional day.
This approach helps ensure sperm are already waiting when the egg is released.
Basal Body Temperature (BBT)
Basal body temperature (BBT) is your body's resting temperature, taken immediately upon waking before getting out of bed. It’s my least favourite way to track fertility… Here’s why
Following ovulation, progesterone causes your body temperature to increase by approximately 0.2-0.5°C.
BBT is excellent for:
Confirming that ovulation occurred
Identifying cycle patterns
Estimating luteal phase length
Helping predict ovulation in future cycles
However, BBT does not predict ovulation because the temperature rise occurs after the egg has already been released.
For this reason, I recommend using BBT alongside cervical mucus observations and LH testing rather than relying on it as your only method of tracking ovulation.
How Often Should You Have Intercourse?
The American Society for Reproductive Medicine recommends intercourse every 1-2 days during the fertile window to maximize pregnancy rates.
Daily intercourse is perfectly acceptable if desired, and studies have shown that it does not reduce pregnancy rates in men with normal sperm parameters.
I also recommend that the male partner avoid going longer than three days without ejaculating. Prolonged abstinence does not improve fertility for most men and may actually reduce sperm quality in some individuals. Regular ejaculation helps ensure a fresh supply of healthy sperm while maintaining excellent sperm counts.
Common Timed Intercourse Mistakes
Many couples unintentionally miss their fertile window. Here are some of the most common mistakes I see in practice:
Waiting until the LH strip is at its darkest before having intercourse.
Only having intercourse once after receiving a positive LH test.
Assuming ovulation always occurs on day 14 (spoiler alert- most people do not ovulate on day 14!).
Ignoring cervical mucus changes.
Stopping intercourse immediately after the first positive LH test.
"Saving up" sperm by avoiding ejaculation for several days before ovulation.
Not considering sperm quality.
Underestimating the impact of lifestyle factors and endocrine disruptors on fertility for both partners (a topic for a future blog post!).
Small adjustments to timing can make a meaningful difference in your chances of conception.
Does Timed Intercourse Actually Work?
Yes. High-quality evidence supports fertility awareness methods when used correctly.
A Cochrane systematic review found that using urinary LH testing modestly increased pregnancy and live birth rates compared with no ovulation prediction. Professional guidelines from the American Society for Reproductive Medicine also recommend intercourse every 1-2 days during the fertile window as the most effective strategy for couples trying to conceive naturally.
Putting It All Together
If you're new to fertility tracking, here's the strategy I recommend to my patients:
Start watching your cervical mucus after your period ends.
Begin LH testing several days before you expect to ovulate (for many people, this may mean starting around cycle day 7 or 8).
As soon as your cervical mucus becomes wetter or your LH strips begin darkening, start having intercourse every 1-2 days.
Continue through your positive LH test and for one additional day.
Track your basal body temperature to confirm ovulation and better understand your cycle for future months.
Remember, the goal isn't to have intercourse on ovulation day- it's to have sperm already waiting when ovulation occurs.
Understanding your body's fertility signs can reduce stress, improve confidence, and help maximize your chances of conception each cycle.
When Should You Seek Additional Fertility Support?
While optimizing timed intercourse is one of the simplest and most effective first steps when trying to conceive, it's important to recognize when additional investigation may be helpful.
If you've been timing intercourse appropriately and pregnancy hasn't occurred, it doesn't necessarily mean you're doing anything wrong—there may be underlying factors affecting fertility that deserve further evaluation.
Current evidence-based guidelines recommend seeking a fertility assessment if:
You are under 35 years old and have been trying to conceive for 12 months without success.
You are 35 years or older and have been trying for 6 months without success.
You have irregular or absent menstrual cycles, which may indicate that ovulation is not occurring consistently.
You have a history of PCOS, endometriosis, thyroid disorders, pelvic inflammatory disease, or previous pelvic surgery.
You have experienced two or more pregnancy losses.
There are known or suspected male factor fertility concerns, such as a history of undescended testes, testicular injury, chemotherapy, or an abnormal semen analysis.
Note: If you have a feeling that something is off, that is reason enough to seek proactive and preventative fertility care. You do not need to wait until you meet a specific timeline to seek support.
Remember that fertility is a team effort. Approximately 30-40% of infertility cases involve male factors, another 30-40% involve female factors, and many couples have contributing factors from both partners. For this reason, a fertility evaluation should ideally include both partners.

Final Thoughts
Trying to conceive can quickly become overwhelming, especially when you're faced with conflicting advice online.
The good news is that understanding when to have intercourse is one of the most impactful changes many couples can make.
By learning to recognize your cervical mucus, understanding your LH tests, confirming ovulation with basal body temperature, and timing intercourse before ovulation occurs, you'll be giving yourself the best possible chance of conceiving naturally each cycle.
